Bisoprolol fumarate is a selective β1-adrenergic receptor antagonist that reduces heart rate, cardiac contractility, and blood pressure, thereby decreasing the heart's workload and oxygen demand. It is available in film-coated tablets of 1.25 mg, 2.5 mg, 3.75 mg, 5 mg, 7.5 mg, and 10 mg. It is used for hypertension, chronic stable angina, heart failure, and certain arrhythmias, either alone or in combination with other cardiovascular medications.

How Concore Works

Bisoprolol selectively blocks β1-receptors in the heart, preventing stimulation by adrenaline and noradrenaline. This leads to slower heart rate (chronotropy), reduced contractility (inotropy), and lower blood pressure, which helps reduce cardiovascular events and mortality in patients with heart failure and reduced ejection fraction. It does not have significant membrane-stabilizing activity or intrinsic sympathomimetic activity at therapeutic doses.

Dosage and Administration

Typical dosing: Once daily, orally, with or without food
Starting dose: Often 2.5 mg or 5 mg depending on the condition and patient response
Titration: Dose may be gradually increased based on tolerance and therapeutic effect.
Tablet handling: Do not crush, chew, or break tablets unless instructed; some tablets can be halved along the score line if partial dosing is required
For heart failure, used in addition to ACE inhibitors, diuretics, and optionally cardiac glycosides

Side Effects

Common side effects include:

Fatigue or tiredness
Dizziness or faintness
Slow heart rate (bradycardia)
Cold extremities (hands and feet)
Rarely, hypotension or worsening heart failure

Patients should report persistent or severe symptoms to their doctor.

Precautions and Contraindications

should not be used in patients with:

Severe bradycardia
Hypotension
Decompensated heart failure
Sick sinus syndrome
Cardiogenic shock
Known hypersensitivity to bisoprolol

Caution is advised in patients with asthma, COPD, diabetes, thyroid disorders, or peripheral vascular disease, and potential drug interactions may occur with other antihypertensives, diabetes medications, asthma drugs, or certain antidepressants.

Storage: Store in a cool, dry place at room temperature, away from direct sunlight and heat. Shelf life is typically 3 years from manufacture
