Irinotecan is a topoisomerase I inhibitor used as a chemotherapy medication. It works by interfering with the action of topoisomerase I, an enzyme that helps unwind DNA during cell division. By inhibiting this process, irinotecan prevents cancer cells from replicating, ultimately leading to cell death. It is commonly administered intravenously.

Main Uses

Colorectal cancer (metastatic), either as a single agent or in combination with 5-fluorouracil (5-FU) and leucovorin (FOLFIRI regimen).
Small cell lung cancer (off-label in some settings).
Other solid tumors in investigational or combination therapies.

Contraindications

Hypersensitivity to irinotecan or any component of the formulation.
Chronic inflammatory bowel disease (IBD) or bowel obstruction (due to severe risk of diarrhea).
Severe bone marrow suppression (neutrophil count <1,500/mm³).
Pregnancy and breastfeeding (due to risk of fetal harm).
