Xabitor (Apixaban)

Description: Apixaban is a blood-thinning medication similar to warfarin, designed to reduce the likelihood of blood clots in patients with atrial fibrillation (an irregular heartbeat) and other stroke risk factors. When a blood clot obstructs an artery, it is referred to as an embolism.

Dosage: Twice per day or based on the prescription of your doctor.

• Stroke Prophylaxis with Atrial Fibrillation
  • 5 mg PO BID
• Postoperative Prophylaxis of DVT/PE
  • Give 2.5 mg PO 12-24 hr after surgery
• DVT or PE Treatment
  • 10 mg PO BID x 7 days, then 5 mg BID

Precautions:  It raises the risk of bleeding and can lead to serious, potentially fatal hemorrhaging. Be vigilant for signs and symptoms of blood loss, and report them immediately or go to an emergency room. Discontinue therapy in patients with active pathological bleeding.

Combining with other drugs that affect hemostasis increases the risk of bleeding, such as aspirin and other antiplatelet agents, other anticoagulants, heparin, thrombolytic agents, SSRIs, SNRIs, and NSAIDs.

Storage: Keep the medication at room temperature, protected from light and moisture. Avoid storing it in the bathroom. If you need to crush the tablet and mix it with water, apple juice, or applesauce, use the mixture within 4 hours of preparation. Ensure all medications are kept out of reach of children and pets. Do not flush medications down the toilet or pour them into a drain unless directed. Properly dispose of this product when it is expired or no longer needed by consulting your pharmacist or local waste disposal company.
