Claryn (Clarithromycin) is used to treat certain bacterial infections of the skin, lungs, sinuses, throat, and ears. Helicobacter pylori-related stomach ulcers are also treated with this medication in combination with other medications.
Indications:
- Respiratory tract infections
- Mild to moderate skin & soft tissue infections
- Otitis media
- Helicobacter pylori eradication.
Dosage:
By mouth, 250 mg every 12 hours for seven days; for severe infections, 500 mg every 12 hours for up to 14 days; for children under eight kilograms of body weight, 7.5 mg/kg twice daily; and for eight to eleven kilograms (1-2 years). 125 mg twice daily; 20-29 kg; 62.5 mg twice daily; 12-19 kg (3-6 years); (7-9 years). 30-40kg (10-12 years): 250 mg twice daily or as needed; 187.5 mg twice daily; or as prescribed by a doctor.
Administration:
Standard Release Tablet & Oral Suspension: May be taken with or without food.
Modified Release Tablet: Should be taken with food. Swallow whole, do not chew/crush

Adverse Reactions:
- Gastrointestinal disorders such as abdominal pain, diarrhea, nausea, vomiting, dysgeusia, dyspepsia
- Increased liver enzymes, hepatocellular hepatitis, and/or cholestatic hepatitis with or without jaundice are all signs of hepatic dysfunction
- CNS effects such as anxiety, dizziness, insomnia, hallucinations, confusion
Special Precaution:
Patients with compromised hepatic or renal function should use caution, and those with severe renal impairment should take lower doses.
Contraindication:
Patients with known macrolide antibiotic hypersensitivity should not use clarithromycin.
Drug Interaction:
May increase blood levels of medicines metabolized by the CYP3A isozyme and inhibit their metabolism:
- alprazolam, astemizole, carbamazepine, cilostazol, cisapride, cyclosporine, disopyramide, ergot alkaloids, lovastatin, methylprednisolone, midazolam, omeprazole, warfarin, pimozide, quinidine, rifabutin, sildenafil, simvastatin, tacrolimus, terfenadine, triazolam, vinblastine; other isozymes of CYP450 eg, phenytoin, theophylline, valproate.
